1.A certificate of degree authenticated by a consulate or a representative of the Republic of China(Taiwan).

2.A copy of transcript authenticated by a consulate or a representative of the Republic of China (Taiwan).

3. Two copies of the application form with a recent photograph attached to each.

4.  Two copies of diplomas obtained (with certified Chinese or English translations if the original is in a language other than Chinese or English) and two copies of complete transcripts in English.

5.Two letters of recommendation.

6. A health certificate (including HIV test and chest X-ray) and valid health and injury insurance for at least 4 months.

7. A study proposal in Chinese or English.
